29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section. (a) Molding Area: On or about 09-24-2024, employees were exposed to caught-in and struck-by type hazards by not utilizing Lockout Tagout procedures during the maintenance of a Newbury 30 Injection Molding Machine, where an inadequately guarded piston was in motion.
