OSH ACT of 1970 Section (5)(a)(1):The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees. a) On or about August 21, 2024 at the jobsite: employees were allowed to operate a Crown (model C51000-50, serial 10047875) forklift in the warehouse, with approximately 100 Martin 60" x 60" Stacking Tire Racks , 5' high, stacked 3 high for a total height of 15', that were not anchored to the floor, exposing employees to struck by and/or crushing hazards.
