1926.21(b)(2) The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury: a) Perch Creek Easement; On or about August 14, 2024, and at times prior to the employer exposed employees to burn hazards in that the employees were engaged in the changing out of hard pressurized lines without adequate training.
