29 CFR 1928.57(a)(6): At the time of initial assignment and at least annually thereafter, the employer did not instruct every employee in the safe operation and servicing of all covered equipment with which he was or would be involved: On or about August 14, 2024, and times prior thereto, at a grain storage silo located at 11682 County Road 691 in Dudley, Missouri, Murphy Grain LLC did not initially instruct employees in the safe operation and servicing of equipment such as but not limited to electrical motors, conveyors, pulleys, gears, and augers. Employees performing this work were exposed to struck-by, caught-in and electrocution hazards when safe practices for maintenance and servicing of equipment, including electrical power de-energization (lockout) were not established.
