At 5560 Page Ave, Saint Louis, MO, 63122, on August 16, 2024, employees were exposed to falls from working at heights greater than six feet on a residential structure without protection from guardrail systems, safety net system, or personal fall arrest system. The employee was working at the edge of the roof without any other employee nearby.
