29 CFR 1926.502(d)(17): The attachment point of the body belt was not located in the center of the wearer's back: a)MK1 Construction, LLC at 3131 North Osceola Street, Denver, CO 80212: On or about and at times prior to August 14, 2024, the employer did not ensure that employees who were working on the steep-pitch church roof were utilizing their personal fall arrest systems (PFAS) correctly, in that employees attached their lanyard to the front of their harness at waist level. This condition exposed employees to a fall hazard of approximately 30 feet to the ground.
