Text For Citation: 01 Item/Group: 001 Hazard:
29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:(a) On or about August 1, 2024, at the previously addressed job site, employees were performing servicing and maintenance of a machine without utilization of hazardous energy control plan that led to an amputation. The OSI Group, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.147(c)(4)(i), which was contained in OSHA inspection number1652586, citation number 1, item number 1 and was affirmed as a final order on 06-15-2023, with respect to a workplace located at 1200 Industrial Drive FORT ATKINSON, WI 53538.
