Text For Citation: 01 Item/Group: 001 Hazard: EXPLOSION
Section 5 (a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to: a) On or about 7/30/87 employees were exposed to a "D" fault-fire, possible explosion from 27,000 volts minimum of 17,500 amps on an energized feeder cable. The feeder cable 9B08 located in a manhole at south Channel Bridge, Howard Beach, NY 11414, was not immediately removed from service and repaired as required in Con Edison EO 1184-3 (1/12/98), Distribution Underground Cable Joint Inspections "C" & "D" Faults. Employees were engaged in blanketing the swollen joint in order to work on another feeder cable. Among other methods, one feasible and acceptable abatement method to correct this hazard is: Remove "D" Fault line, do not enter manhole without deenergizing the feeder cable.
