Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The %% employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to: a. JOBSITE: South side of barn, Rt. #5, 6078 East Genesee St. Fayetteville, N.Y.; (3) Three employees were working from a ladder jack scaffold installing roofing material and roofing brackets when one sup- port ladder of the scaffold broke causing the (3) employees to fall approx. 20 feet to the ground below. Among other methods, one feasible method to reduce this hazard is to establish and enforce adequate safe operating procedures, procedures such as those stipu- lated in American National Standards - Scaffolding Safety Requirements (ANSI A 10.8 - 1988). Elements essential to an acceptable safe operating procedure include at a minimum: 1) Ladder jack scaffolds erected with a fabricated scaffold plank to be limited to no more than two persons on the plank, but only one person to stand on each half of the planks span. 2) Ladders will be Type I (250 pound rated) or Type 1A (300 sound rated).
