29 CFR 1910.23(b)(2): The employer did not ensure that ladder rungs, steps, and cleats were spaced not less than 10 inches (25 cm) and not more than 14 inches (36 cm) apart, as measured between the centerlines of the rungs, cleats, or steps. On or about July 17, 2024, and at times prior to, in the grain elevator boot pit access; employees use a fixed ladder, with rungs spaced more than fourteen (14) inches apart, to enter and exit the boot pit area.
