29 CFR 1910.178(a)(6): The employer did not ensure that all name-plates and markings were in place and maintained in a legible condition: (a) Employees were exposed to struck-by hazards. Employees engaged in material handing activities using the Caterpillar forklift number 10, were exposed to stuck-by hazards in that the following markings were not in place and legible: 1) The control lever that shifts the mast side to side did not include markings. 2) The control lever that moves the mast forward and reverse did not include markings. 3) The control lever that tilts the mast did not include markings. (b) Employees were exposed to struck-by hazards. Employees engaged in material handing activities using the Caterpillar forklift number 99, were exposed to stuck-by hazards in that the following markings were not in place and legible: 1) The control lever that moves the mast forward and reverse did not include markings. 2) The control lever that tilts the mast did not include markings.
