29 CFR 1910.178(a)(4): Modifications and additions which affect capacity and safe operation of powered industrial truck were performed by the employer without the manufacturer's prior written approval: Pride California Dry Dock: On July 16, 2024, the employer did not ensure that manufacture approval (variance) was received and documented before modifying the forks on a JLG Skytrack Telehandler. This condition potentially exposed employees to crushing, struck-by and/or amputation hazards.
