29 CFR 1910.178(l)(2)(ii): The employer did not ensure that each operator had successfully completed the training consisting of a combination of formal instruction (e.g., lecture, discussion, interactive computer learning, video tape, written material), practical training (demonstrations performed by the trainer and practical exercises performed by the trainee), and evaluation of the operator's performance in the workplace. (a) 3910 Oxford Road, Gettysburg PA, Shipping and Receiving work area - On or about July 3, 2024, the employer did not ensure that an employee had formalized instruction to safely use the walk/ride Barrett powered pallet jacks. The employee was exposed to struck-by hazards. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
