29 CFR 1926.502(i)(3):All covers were not secured when installed so as to prevent accidental displacement by the wind, equipment, or employees: a) At 3062 SR 143, Elmore AL: On or about June 25, 2024 and at times prior thereto; the employer exposed employees to fall hazards of approximately 26 feet and 6 inches to the concrete below in that employees were performing roofing activities around a hole for which the cover was not secured to prevent accidental displacement.
