OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to injuries caused by the operation, collision, and/or rollover of powered industrial trucks and the failure to wear seatbelts properly: On or about June 27, 2024, at location W4892 County Road O, Appleton, Wisconsin: Employer failed to ensure that forklift operators were utilizing an operator restraint system while moving material around the facility.
