29 CFR 1926.28(a): Appropriate personal protective equipment was not worn by employee(s) in all operations where there was exposure to hazardous conditions a) Jobsite, 1023 Pittsfield Alley, New Brighton, PA 15066: On or about June 20, 2024, appropriate personal protective equipment was not worn by the employee where there was exposure to hazardous condition. An employee was not wearing a seatbelt while operating a John Deer 135G Excavator when it rolled down a slope at a 36-degree angle and suffered a fatality when inside the cab.
