29 CFR 1910.212(a)(3)(ii):Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s): a) On or about June 15, 2024, and at times prior, employees in the meat department were exposed to amputation hazards where the Bird Meat Grinder (Model 346-3; Serial # 31494) was operated without the tray and guard assembly in place. On this date, an employee suffered the amputation of a finger after reaching into the point of operation.
