29 CFR 1910.212(a)(3)(iii): Special hand tools for placing and removing material(s) was not provided to permit easy handling of material without the operator placing a hand in the danger zone: On or about 06/17/2024, employees were exposed to caught-in-between hazards while working at the manufacturing facility located at 5490 County Road 33 in Koshkonong, MO. Employees remove debris from the inside of a material mixer chute which was not equipped with a special hand tool to prevent an employee's hands from coming into the danger zone.
