OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm in that three employees were exposed to crushing and struck-by hazards while removing two lifting hooks from pre-assembled half column formwork components that was left free-standing and unsecured: a) Jobsite located at 3615-35 Chestnut St. Philadelphia, PA 19104 - On or about June 17, 2024 - The employer failed to protect employees against crushing and struck-by hazards in that the pre-assembled half column Doka Framax Xlife formwork was left free standing and unsecured while the lifting hooks were removed.
