29 CFR 1926.651(j)(2): Protection was not provided by placing and keeping excavated or other materials or equipment at least 2 feet (.61m) from the edge of excavations, or by the use of retaining devices that were sufficient to prevent materials or equipment from falling or rolling into excavations, or by a combination of both if necessary. (a) On or about June 7, 2024, at a jobsite located at or near 7897 88th Ave, Pleasant Prairie, WI 53158, employees working between seven (7) and 10 feet below ground level in an excavation, were exposed to struck-by hazards from tools and equipment, to include a Stihl Concrete Saw, Honda 2200 Generator, and other work related items located directly above and at the South side edge of the excavation, near the egress ladder.
