29 CFR 1926.251(a)(1): Rigging equipment for material handling was not inspected prior to use on each shift and as necessary during its use to ensure that it was safe, or defective rigging equipment removed from service a. At the intersection of International Speedway and Atlantic Avenue in Daytona Beach Florida. On or about May 28, 2024, in the 6-feet deep trench, the employer exposed employees to struck-by hazards from the large plastic drainage pipe sections in that the pipe was moved with rigging and a lifting hook attached to the excavator bucket that was missing the flipper safety latch, exposing employees to a struck-by hazard.
