29 CFR 1926.100(a): Employees were not protected by protective helmets while working in areas where there was a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns: At a residential work site located at 4610 Forrest Ave, Kansas City, Missouri. An employee was exposed to struck by hazards from rocks and other debris by not wearing personal protective equipment in the form of a hard hat while working in an unprotected trench approximately 15 feet in depth.
