29 CFR 1926.21(b)(2): The employer shall instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his work environment to control or eliminate any hazards or other exposure to illness or injury. a. Located at the job site at 2062 Broccoli Dr., Hilliard, Ohio 43026: On or about May 20, 2024, the employer did not ensure that employees were adequately trained on subjects including, but not limited to: trenching and excavation safety including inspections and safe operation of the CAT 330F hydraulic excavator. Employees were thereby exposed to serious injuries while performing site preparation and water line installation work.
