29 CFR 1926.20(b)(2): The employer did not initiate and maintain programs which provided for frequent and regular inspections of the job site, materials and equipment to be made by a competent person(s): On or about May 15, 2024, employees were exposed to trip and fall to the same level hazards while having access to a walking/working surface which contained floor holes and the employer had not ensured the person conducting the worksite inspection was knowledgeable in the standard/requirements to identify and eliminate the hazards related to floor holes in the walking/working surface.
