29 CFR 1910.28(b)(3)(i): The employer did not ensure that each employee is protected from falling through any hole (including skylights) that is 4 feet (1.2 m) or more above a lower level by one or more of the following methods listed in 29 CFR 1910.28(b)(3)(i)(A)-(D): a) Leadec Corp located at 799 Abbe Road, Sheffield Lake, Ohio, 44054: On or about April 26, 2024, the employer failed to ensure that employees were protected from falling through the hatch openings of the wellhouses of the phosphate tank line in the Paint Department. Employees were exposed to falls of approximately 4.5 feet while performing tank cleaning activities.
