(a) On or about May 1, 2024, an employee operated the telescoping boom and basket for the Altec telecommunication truck (# 2193) moving the conductive basket into or closer than permitted to the overhead energized power lines which resulted in the fatal electrocution of one employee and the shock of another employee.
