29 CFR 1910.272(e)(1):The employer did not provide training to employees at least annually, new employees prior to starting work, and when changes in job assignments would expose them to new hazards in the grain handling facilities: In the McPherson/#1 grain bin, employee(s) performing housekeeping activities were exposed to amputation hazards. The employer did not provide initial training to new employee(s) prior to entering grain bins. Instance of failure as a result of inadequate or lack of training include but are not limited to: 1. Failure to recognize the potential for atmospheric hazards and requirement to test the air quality of a grain bin prior to entering. 2. Failure to perform either entrant or attendant related duties during grain bin entry. 3. Failure to recognize the requirement to lock out the sweeper and auger to prevent mechanical hazards during grain bin entry. 4. Failure to recognize the requirement of the bin entry team to fill out a bin entry permit that needs to be reviewed and made available to the entry team throughout the bin entry.
