29 CFR 1910.23(d)(4): The employer did not ensure that the side rails of through ladders extend at least 42 inches (1.1 m) above the top of the access level served by the ladder: On or about April 26, 2024: Employee(s) accessing underground conveyors with a fixed ladder at or about 101 North Franklin Street, Brunswick, Nebraska were exposed to fall hazards. The employer had employee(s) moving in and out of a boot pit on a fixed ladder where the side rails did not extend at least 42 inches and the height from the top of the ladder to the floor below exceeded 10 feet.
