29 CFR 1910.132(d)(1)(i): The employer did not select and have each affected employee use, the types of personal protective equipment that would protect the affected employee(s) from the hazards identified in the hazard assessment: (a) On or about April 16th, 2024, at 1919 Northwest 19th Street, Fort Lauderdale, Florida, an employee was exposed to amputation and caught-between hazards while cutting meats with a band saw due to the employee wearing string knit gloves cut resistant near the equipment's blade and lacking a clear view of the task being conducted when using eyewear that was not anti-fog type. (b) On or about April 19th, 2024, at 1919 Northwest 19th Street, Fort Lauderdale, Florida, an employee was exposed to amputation and caught-between hazards while cutting meats with a band saw due to the employee wearing string knit gloves.
