29 CFR 1910.268(b)(7): The employer did not ensure that no employee approached or took any conductive object closer to any electrically energized overhead power lines and parts than prescribed in Table R-2 or in (i) - (iii) of this section: (a) Worksite, just north of 319 Long Road, East Berne, NY: On or about April 19, 2024, an employee was performing node maintenance on a telecommunication line from a Altec AT235 non-insulated articulating aerial truck when he came into contact with an overhead 4.8 kV powerline. The employer did not ensure that the employee maintained a minimum approach distance of 24 inches from the energized power lines (per 1910.268 table R-2), resulting in electrocution hazards.
