29 CFR 1926.501(b)(13):Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): (a) On or about April 16, 2024, located at 1407 Mulberry Street, Hartford, WI 53027, the employer failed to ensure employees tasked with tear-off operations and installation of underlayment were protected by guardrail systems, safety net systems, and/or personal fall arrest systems, exposing the employees to a fall hazard of approximately nine and a half (9.5) feet. Juan Carlos Alvarez Govea DBA JC Roofing Contractor LLC was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 1715890, citation number 1, item number 1 and was affirmed as a final order on April 10, 2024, with respect to a workplace located at 3800 Sunnycrest Drive, Brookfield, WI 53005. Juan Carlos Alvarez Govea DBA JC Roofing Contractor LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(1), which was contained in OSHA inspection number 1607684, citation number 2, item number 1 and was affirmed as a final order on October 19, 2023, with respect to a workplace located at 2700 Golf Road, Delafield, WI 53018. Juan Carlos Alvarez Govea DBA JC Roofing Contractor LLC was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 1470124, citation number 1, item number 1 and was affirmed as a final order on April 24, 2020, with respect to a workplace located at 1722 Hunter Road, Waukesha, WI 53189.
