29 CFR 1910.178(a)(4):Modifications or additions which affect capacity and safe operation of powered industrial truck were performed by the user without the manufacturer's prior written approval: a) At Witzco Challenger Trailers, located at 6101 McIntosh Road Sarasota, FL in the Gooseneck section of the Finish Shop: On or about April 12, 2024, the employer exposed employees to struck-by and crush hazards in that extensions were permanently welded to the forks without the manufacturer's written approval.
