29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used. Scaffold frame was used as a means of access: a) 4701 Pine St, Philadelphia, PA 19143 - Exterior Scaffold, West side of building - On or about April 11, 2024, the employer did not ensure employees were provided safe means to access and egress the scaffold. Employees utilized the scaffold supports to access the 2nd level of the scaffold platform, thereby exposing employees to fall hazards of approximately 13.5 feet. b) 4701 Pine St, Philadelphia, PA 19143 - Exterior Scaffold, South side of building - On or about April 11, 2024, the employer did not ensure employees were provided safe means to access and egress the scaffold. Employees utilized the scaffold supports to access the 1st level of the scaffold platform, thereby exposing employees to fall hazards of approximately 6 feet.
