OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees from struck by and crush by hazards: a) Location: Loading Docks #1-57 On or about April 7, 2024, employees were not protected from struck by and crush by hazards when working behind the vertically stored dock plates of the Powerramp VS Series dock levelers and methods to secure the dock plate from falling had not been taken. b) Location: Loading Dock #8 On or about April 7, 2024, employees were not protected from struck by and crush by hazards when the manufacturers published quarterly, and annual maintenance schedule was not followed for the Powerramp VS Series dock leveler. c) Location: Loading Dock #8 On or about April 7, 2024, employees were not protected from struck by and crush by hazards when operating and working behind the vertically stored dock plates of the Powerramp VS Series dock levelers without being trained on the functions and hazards of the dock leveler.
