29 CFR 1910.132(a): Protective equipment was not used, when necessary, whenever hazards capable of causing injury and impairment were encountered: a) Frazer & Jones, LLC, 3000 Milton Avenue, Solvay, New York, 13209. On or about 5/09/2024 the employer failed to ensure that a Foundry employee oxygen lancing the tundish nozzle, used the appropriate personal protective equipment, in that the employee did not wear protective clothing, such as, but not limited to aluminized insulated pants. The employee's pants caught on fire, causing a burn to the back leg of the employee. Abatement certification must be submitted for this item
