29 CFR 1910.147(c)(4)(i):Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: a)Tube cutting area; On or about March 27, 2024 and times prior employees were exposed to amputation and crushing hazards in that the employer failed to ensure procedures were utilized for the control of potentially hazardous energy when employees performed maintenance tasks such as, but not limited to, changing of the die blocks on the Komatsu Tube Press TP3. Madix was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.147(c)(4)(i), which was contained in OSHA inspection number 1659040, citation number 2, item number 1 and was affirmed as a final order on September 06, 2023, with respect to a workplace located at 500 Madix Drive, Eclectic, AL 36024. Madix was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.147(c)(4)(i), which was contained in OSHA inspection number 1623363, citation number 1, item number 1 and was affirmed as a final order on December 23, 2022, with respect to a workplace located at 500 Airport Road, Terrell, TX 75160. Madix was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.147(c)(4)(i), which was contained in OSHA inspection number 1403315, citation number 1, item number 1 and was affirmed as a final order on October 8, 2019, with respect to a workplace located at 500 Madix Drive, Eclectic, AL 36024.
