Text For Citation: 01 Item/Group: 001 Hazard:
OSH ACT of 1970 Section (5)(a)(1):The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees utilizing a field modified crane and unrated below the hook lifting device were exposed to crushing hazards: a) 275 Lower Morrisville Road, Fallsington, PA 19054 - 101 Building: On or about May 29, 2024, the crane support columns were lengthened without prior approval from the manufacturer. b) 275 Lower Morrisville Road, Fallsington ,PA 19054 - 101 Building: On or about May 29, 2024, the spreader bar used in conjunction with the underhung crane was not load tested as a lifting device. Among other methods, feasible and acceptable means of abatement include, but are not limited to: ASME B30.11 11-1.3.5 Modifications The crane and monorail systems may be modified or rerated, provided such modifications and the supporting structure are analyzed by a qualified person or manufacturer of the equipment. A rerated system or one whose load-supporting components have been modified shall be tested in accordance with Section 11-2.2. The new rated load shall be displayed in accordance with Section 11-1.1 ASME B30.20 20-1.3.8.2 Load Test Prior to initial use, all new, altered, modified, or repaired lifting devices should be tested and inspected by or under the direction of a qualified person and a written report be furnished by such a person, confirming the load rating of the lifter. The load rating should not be more than 80% of the maximum load sustained during the test. Test loads shall not be more than 125% of the rated load unless otherwise recommended by the manufacturer. Test reports should be available.
