29 CFR 1910.178(l)(4)(iii): An evaluation of each powered industrial truck operator's performance was not conducted at least once every three years. At the workplace located at 2925 Industrial Road, Junction City, Kansas, the employer failed to verify the power industrial truck operator training. It had been more than 16 years since the last powered industrial training for one operator, thus exposing employees to struck-by powered industrial truck hazards.
