29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of hazardous energy when employees were engaged in alignment and maintenance operations of the dock plates. A) US Foods, 1051 Amboy Ave, Perth Amboy, NJ 08861 - Loading Dock Area: The employer did not ensure that all employees utilized lockout/tagout procedures when fixing jams of dock plates in the bay area of the facility. This condition exposed employees to struck by hazards. Condition observed on or about 3/27/2024.
