29 CFR 1910.28(b)(3)(i): Holes. The employer must ensure each employee is protected from falling through any hole (including skylights) that is 4 feet (1.2 m) or more above a lower level by one or more of the following required under 29 CFR 1910.28(b)(3)(i)(A) to (D), covers, guardrail system(s), travel restraint system(s), or personal fall arrest systems respectively: On or about March 9, 2024 and thereafter; at an establishment located at 2900 East Richmond Street, in Shawano, Wisconsin; on the third level of the Laempe L150 Core Machine there were two open holes during cleaning activities exposing employees to slip, trip, and fall hazards.
