29 CFR 1904.29(a): A log of all Work-Related Injury and Illnesses (OSHA Form 300), and/or the Summary of Work-Related Injury and Illnesses, (OSHA Form 300-A), and/or the Injury and Illness Report (OSHA Form 301) or equivalent forms were not kept by the establishment: a) Technotrix, Inc. located at 1407 Huntington Dr., Calumet City, IL 60409: On or about December 1, 2023, the employer failed to keep injury and illness records (OSHA Form 300 or similar), summary of injury and illnesses (OSHA Form 300-A or similar), and an injury and illness report (OSHA Form 301 or similar) for a recordable injury that occurred. One employee suffered a shoulder injury resulting in days away from work and/or a job transfer or restriction.
