Text For Citation: 01 Item/Group: 001 Hazard:
Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees who were exposed to struck-by hazards: (a) On March 11, 2024, at Cyclone Rig 38 on Continental Chase 5-18H near 113th Ave SW Killdeer, ND, employees were exposed to being struck-by the drill line while performing slip and cut operations, when the top drive was not secured prior to releasing the dead-man anchor causing the top drive to fall. As the top drive fell, the slack in the drill line reacted uncontrollably resulting in struck-by hazards. Note: Among other methods, feasible and accepted methods to correct this hazard would be to: 1) Follow the work sequence of Cyclone Drilling JSA#9.1 Cut and Slip Drill Line and ensure the top drive is secured prior to releasing the dead-man anchor. 2) Ensure that the guards on the top drive are in place and working properly in accordance with API Recommended Practice 54 section 7.5 which states: "The hoisting line should not be removed from the hoisting drum until the traveling block is rested on the rig floor or held suspended by a separate purpose-built, support device."
