29 CFR 1910.30(a)(3)(iv): The employer did not train each employee in the correct use of personal fall protection systems and equipment specified in paragraph (a)(1) of this section, including, but not limited to, proper hook-up, anchoring, and tie-off techniques, and methods of equipment inspection and storage, as specified by the manufacturer. a) 10 East Park Drive, Westampton, NJ: Employees were exposed to fall hazards of approximately 8 feet to the ground below when loading carpets onto a the work platforms of a Clark Forklift Model #TMX25 and Model #S25CL. The employees improperly attached the lanyard to the forklift by wrapping it around the frame and not utilizing a proper anchor point. The employer did not provide training on the correct use of fall protection and how to properly anchor to the lift for protection from falls. Observed on or about 02/20/2024. NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.
