29 CFR 1910.147(f)(3)(ii)(D): Each authorized employee did not affix a personal lockout or tagout device to the group lockout device before working on the machine or equipment: a) At 353 Ashford Industries Rd. Ashford, AL.: On or about February 14, 2024, and at times prior, the employer exposed employees to amputation hazards in that employees were allowed to perform maintenance activities on the One-Man Saw Model 237 S/N 12-1111-22 without group lockout/tagout being performed.
