29 CFR 1910.147(c)(4)(i):Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: a) Wieland Chase LLC. on or about January 23, 2024, at the address of 14212 Selwyn Dr. Montpelier, OH: Employees clearing jams in stem, and extruder were exposed to caught in injuries in that the employer did not develop, document, or utilize procedures to verify and control the hazardous energy created by the moving components of the Hydraulic extrusion press on line #1. b) Wieland Chase LLC. on or about January 23, 2024, at the address of 14212 Selwyn Dr. Montpelier, OH: Employees performing set up activities including but not limited to stem changes for product changeover, were exposed to caught in injuries in that the employer did not develop, document, or utilize procedures to verify and control the hazardous energy created by the moving components of the Hydraulic extrusion press on line #1. c) Wieland Chase LLC. on or about January 23, 2024, at the address of 14212 Selwyn Dr. Montpelier, OH: Employees performing cleaning activities in the press #1 carriage pit, were exposed to caught in injuries in that the employer did not develop, document, or utilize procedures to verify and control the hazardous energy created by the moving carriage of the Hydraulic extrusion press on line #1.
