Machine guarding was not provided to protect operator(s) and other employees from hazards created by in-going nip points, rotating parts, and/or flying chips and sparks: Location: a) 2nd Floor, 341 9th Avenue, New York, NY 10014 On or about January 26th, 2024, an employee, who was sorting mail on the APBS3 machine, was exposed to an amputation hazard from being caught in or struck-by exposed rotating parts. Mail items were stuck in an opening on the conveyor belt, and the employee retrieved the stuck mail by hand while the machine was in operation. 2nd repeat: U.S. Postal Service was previously cited for a violation of this occupational safety and health standard or its equivalent standard, 29 CFR 1910.212(a)(1), which was contained in OSHA inspection number 1555050, citation number 2, item number 2 and was affirmed as a final order on March 28, 2022, with respect to a workplace located at 3701 West Wendover Avenue, Greensboro, NC 27495. U.S. Postal Service was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.212(a)(2), which was contained in OSHA inspection number 1203272, citation number 1, item number 1 and was affirmed as a final order on March 20, 2017, with respect to a workplace located at 660 Westfield Road, San Francisco, CA 94128.
