Grinding Department: Grinder 3168 - On or about February 1, 2024, the employer failed to provide training to each authorized employee performing service and/or maintenance to machinery or equipment to ensure the employee acquired the knowledge and skills required for the safe application, usage and removal of energy control devices. Machine operators were exposed to caught-in hazards to their hands and fingers while performing screw changes on the grinding wheel.
