1910.178(l)(6): The employer shall certify that each operator has been trained and evaluated as required by this paragraph (l). The certification shall include the name of the operator, the date of the training, the date of the evaluation, and the identity of the person(s) performing the training or evaluation: Facility Laydown Area: On or about January 23, 2024 and times prior, the employer exposed employees to struck-by hazards, in that powered industrial truck (PIT) recertification records for employees operating the Toyota PIT, (model 7FDKU40, serial number 60926) to move waste containers in and out of the paint area were not kept compliant.
