On or about January 18, 2024, and at times prior thereto, Instance a. In the Bay 1 pit area, employees stood on a metal grating with an uncovered hole while changing vehicle motor oil. Instance b. In the Bay 2 pit area, employees stood on a bent metal grating while changing vehicle motor oil that created an opening and trip hazard.
