Text For Citation: 01 Item/Group: 001 Hazard:
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish to each of the employees a place of employment which was free from recognized struck-by hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to struck-by hazards: (a) On or about January 5, 2024, at the machining area, employees were exposed to struck-by hazards, namely the door of the vertical lathe that failed during the machining process and the part that was then released from the machine. Abatement amended to read as follows: Among other methods, one feasible and acceptable abatement method to correct this hazard includes: (1) Retraining all employees in enforcement of the employer(s) guidelines and requirements, as stated in the Guidelines for jaw selection, soft jaw cutting and chuck pressure. Specifically: (a) The employer shall follow and enforce its Guideline requirement that the final set up be inspected by a supervisor prior to the start of the machining process; and (b) The employer shall follow and enforce its Programming guideline that all programs be checked and edited prior to sending to the machine and that any program changes required at the machine be approved by a supervisor prior to the operation of the machine by the operator; and (c) ANSI B11.22-2002 (R2020) American National Standard; Safety Requirements for Turning Centers and Automatic, Numerically Controlled Turning Machines; and (d) The user shall enforce machine set up procedures in accordance with the recommendation(s) of the supplier(s) of the subject machine and software.
